Attention:  Players Attending Tryouts

All players MUST have the following forms completed upon check-in. 
Please download the following forms and have filled out to help expedite your check-in process.

Consent To Treat - Medical History
Waiver Form - Mouth Guard
Waiver Form - Helmet
Claim Form
Very Important:  If you have a 08-09 USA Hockey registration that expires on Aug. 31, 2009 you will need to update your membership and show proof BEFORE you can tryout.

USA Hockey’s junior council rules that your registration needs to be updated to participate in any tryouts for a junior hockey program after June 1, 2009.
